Payment at Time of Service: All copays, deductibles, balances, and/or coinsurance are required at the time of service without exceptions.

Insurance Payment and Responsibility: If insurance payment is not received within 30 days of the last visit or if your claim is deemed not medically necessary, you will be responsible for the entire visit cost.

Verification of Coverage: While we accept most insurances, coverage verification does not guarantee payment. You are personally responsible for non-covered or denied services, and understanding your insurance policy's benefits and coverage is essential.

Out-of-Network Services: If we are considered out of network, you will be financially responsible for the entire visit cost.

Accepted Payment Methods: Payment is due at the time of service, and we accept most major credit cards. Unfortunately, checks are not accepted. We can provide a Super Bill for potential insurance reimbursement.

Notification of Changes: Notify our billing department within 72 hours of your scheduled appointment for any changes to payment methods or insurance since your last visit. Failure to do so may result in appointment rescheduling.
