No Call No Show Policy

We have a strict No-Show/Late Cancel/Appointment Policy. We give patients ample time to cancel and/or reschedule appointments by using our notification system and/or calling you to reschedule your appointment. This is a courtesy that we have incorporated into our practice to give patients time to plan their appointments.

 

It is the patient's responsibility to remember appointments. It is common courtesy to let us know if you are not able to make your appointment as we have set aside time for you to be seen and evaluated by your provider.  

 

If you cannot make your appointment, we would appreciate a phone call to let us know so we can schedule another patient that may need to be seen in your designated appointment slot.

Initial visit:

You must cancel 72 hours prior to your initial visit. If you fail to do so, we charge a fee of $250.00 which is non-refundable. 

 

You will be charged the full amount of the session for any and all No Shows/Late Cancellations. You may not reschedule until this fee is paid in full. You may only reschedule once.

 

Established Patients:

If you fail to attend a follow-up visit or cancel with less than 48-hour notice, you will be charged a $125.00 no-show/late cancellation fee.  

Once you have paid the no-show/late cancellation fee, you will be permitted to reschedule your appointment.

You will be charged the total amount of the session for any and all No Shows/Late Cancellations

If you have more than two no-shows, late cancellations, or reschedules within the calendar year, you will be referred to a more suitable provider from the practice.

All minors must be accompanied by a parent and/or adult that is authorized to attend any appointments.  No Exceptions!  We value each and every one of our patients and follow all state and federal guidelines in regard to treating minors.
